Re: CC or Ruptured??
It could be either/or. Without running some tests or getting an MRI, there's no knowing really. I believe a rupture can also cause pain and for the breast to become misshapen. She's doing the right thing by seeing her surgeon.

Re: CC or Ruptured??
It's impossibe for us, her or her surgeon to know if she has a ruptured implant unless she has an ultrasound scan, or better still an MRI.
Because she had her surgery privately, she will be required to pay for the scan herself.
However...any Breast pain or changes in breasts, shouldn't just be written off as a complication due to implants and should be fully investigated. She really should see her GP first after so long after surgery so she can first rule out anything else that might be going on. He GP will likely then organise an ultrasound scan for her through the Breast clinic which will be free. If they feel further investigation is needed she'll likely then have an MRI. Your friend will be told the results of the scan. This includes any sign of rupture or CC.
As she's 3 years post-op she should also find out whether or not she's still covered by her aftercare for ruptures or CC.
If there's anything regarding her implants on the scan she can request her consultant send on the scan to her clinic for them to take a look at. If she's within her aftercare and she has a 'clinical diagnosis' (this will be confirmed if anything is there with the scan)
From a guess though, I'm with you....your description does sound like it could be CC, especially from the description of the Breast being 'out of shape'. The capsule surrounding the implant contracts squeezing the implant which could cause the Breast to distort as you mentioned. Xx
