Overs are placed over the chest muscles and unders are placed under. There are also part unders too which are half under and have over.
Profiles give the projection of the implant. So high stick out more than moderate. Have a look at this [You must be registered and logged in to see this link.]
it explains better than me. x

If you look at the diagram above hun, the left shows the implant 'over the muscle, this involves the surgeon making an incision (transaxillary incision (armpit incision), TUBA (navel incision), inframammary crease (under the breast in the crease) or areola (nipple)) and placing the implant in place with as little trauma to the breast as possible.
Completely under the muscle, is very rare and only ever really used for reconstructruction surgery, the most common placement used which those who say the have 'unders' have is a 'dual plane or partial placement as you can see from the pic on the right. This involves the same choice of incision as above and then the surgeon having to disect the muscle partially, this gives coverage to the muscle at the top of the breast leaving the bottom of the implant free and released.
The difference between moderate and high profile is the width and projection babe, the higher the profile the less width you'd have but the more forward projection you'd have babe. xx
