Lots of forms??
Are there lots of forms to fill out at your 1st consultation?

Ermmm... it seems such a long time ago.
I seem to remember having to fill in basic name, address, contact number details whilst waiting in the waiting room before my consult. Oh yes and also any previous operations or illnesses... basically anything which may be relavent to having a BA. I don't think they took long fill out.

I did'ent fill any forms in myself, my p.s. just asked my medical history and filled them in himself (be thankful you dont have to think back as long as me) Like AngelPie it seems such a long time ago!!! He was filling in forms, taking notes as we went along, measuring me, asking me my measurements, requirements, look I wanted, what size I went to whilst breastfeeding etc. Sorry, not much help, maybe one of the more recent girls could be more help?

Not really, I just had to do 1 there and bring in one they'd already asked me to fill in..x

I filled out a medical history to which i ticked 'no' to about 30 things lol then i had to fill out gp details, address etc x

i had to fill out medical history ect and give permission to contact my doctor and of course payment details

At my first consult i had to fill in 1 form.. name, address, medical history etc, then they filled in another form for me with the prices of what i wanted done on it and gave it to me in a folder with other info on the hospital, the surgeons, etc x
