Overs/unders - decide in theatre?!
Hi ladies.
So I had my first proper consultation with a surgeon today (one from MYA). She said I could have implants either over or under, but she wouldn't know which she'd want to go with until I was on the operating table! Is this usual? It's unsettled me a bit. I know sometimes they take a couple of different sizes of implant into theatre with them and decide which is best, but I thought placement would be something decided ahead of time. Has anyone else just left it as a "surprise" for after surgery?

I had overs as didn't want a lift. Had sagging due to breast feeding twice so didn't really have a choice. I would say that placement is definitely one to agree on before you go into surgery through. Have you researched under vs over? X

Re: Overs/unders - decide in theatre?!
I totally understand where you're coming from! My surgeon did EXACTLY the same! It made me really nervous as I went in not knowing over/under or whether it would be 300 moderate or 300/330 high profile but it was the best decision. You have to totally trust the surgeon but from what I understand is that overs are favoured for a more natural placement (it's where your breast tissue would be located if you had it) but some people don't have enough to cover the implant, hence under. Sometimes this is obvious, sometimes it's not and you're borderline. I didn't fill a 34aa preop (although was in the wrong size my band is 30) so flat as a pancake, but after op I was told I had just enough tissue to cover, it was just very flat!!! He opened me up, had a look, and then decided. In the end he went subfascial (which I didn't know existed pre op) where they go over the muscle but under the very top fascial layer of the muscle to give a little extra support and to ensure the implant is as deep as possible without lifting muscle. He also said my muscles were thick so didn't want to mess with them and he felt over the years they would push my implants down and apart. So yes it feels unusual and nerve-wrecking but it paid off for me. That said, make sure you get 3 opinions and you trust your surgeon. 2 other surgeons told me unders, but I totally trusted the one I went with. There's an interesting thread somewhere on here. I'll try and link it. Also make sure you have told the surgeon the look you want using photos!

It's not 'usual' persay, but it's not wrong. Like mammamia said, sometimes it's not immediately obvious to the surgeon, and they need to dig around to see what would be best. Personally, I'd trust this over a surgeon that would say they only do one sort of placement, that would means they're only looking out for themselves, rather than you. That's not to say another handful of surgeons wouldn't all say the same placement. Be sure to get some more opinions, but if you like this surgeon, and are prepared to trust them implicitly to achieve the results you want, then it's fine

Re: Overs/unders - decide in theatre?!
It was only my first proper surgical consultation - I've had a chat with another surgeon but we didn't discuss placements etc. then, it was just to see if I was a suitable candidate for the procedure.
Thanks for the link [You must be registered and logged in to see this link.] - I'm actually booked to see James McDiarmid next week so will see what his opinion is.
I think part of the issue is I don't feel I clicked with the surgeon I saw yesterday. She was pleasant enough, but my husband commented that she didn't seem as confident as the surgeon I had the chat with the other week.

Re: Overs/unders - decide in theatre?!
These ladies know more than me but I had a few consultations and all of them knew what to do except the lady surgeon I saw at MYA! She didn't know if it should be over or under either and wanted me to drive all the way back in to get a second opinion from another surgeon at MYA that was more experienced. Immediately I lost all trust in her so knew she wasn't the surgeon for me. You have to trust your surgeon, whoever you go with. I don't want to put you off but just get some more opinions and do some research on the placements and surgeons (as the other ladies have already said) good luck

I'm most excited about my consultation next week with Mr McDiarmid's reputation on here. I just hope, if I want to go with him, that I won't be stuck waiting ages for surgery!

Re: Overs/unders - decide in theatre?!
I'm nearly 4 weeks post op and up until op day I was told I would be having unders as I had nothing up top, we never decided on size, my surgeon gave me a rough idea of implant sizes but he said he would try sizers on my in theatre and go with what he thought looked best on me with what I had told him I wanted to achieve! On the morning of the op he saw and thought he may be able to do overs instead (admittedly I have put on a little bit of weight since my last consult but only 4lbs!) anyway he said he'd see how he got on but I wouldn't know my size or whether I had overs or unders until I woke up!
I woke up with 420 overs and I'm very pleased! I left it to his judgment and he got it perfect for my body and what I would like the final result to be!

Re: Overs/unders - decide in theatre?!
My surgeon said unders for me from the start as I had very little breast tissue, but on the morning of the surgery he said if he thought overs would be possible when he went in there then he would go with that as recovery time would've quicker for me. He also took 2 different sizes in to try to see which would look best on me, with my permission of course. I trusted his judgement completely and told him so. I ended up with 430cc unders, and I love the look of my result, it's just what I hoped for.
However if he was unsure from the start and wanted a second opinion from a more experience surgeon I would have probably run for the hills - but that's just me. As the other ladies said try and get at least 3 consultations with different surgeons, with one of them preferably a BAAPS or BAPRAS registered surgeon if you can. You really need to have full confidence in your surgeon.
Good luck with your journey, hope you find your perfect match xxx

[You must be registered and logged in to see this link.] it's great to hear your experience with the surgeon I'm seeing in a few days. His response I can more understand, but not knowing from the outset does unsettle me.
[You must be registered and logged in to see this link.] thanks for sharing your experience as well. I guess having complete trust in your surgeon is the most important thing! I didnt have that with the lady I saw this week.
Looking forward to my next consultation, and hoping he's the one (and doesn't have a long waiting list for surgery!)
