Consultation Questionnaire - Help! Empty Consultation Questionnaire - Help!

Can anyone tell me how to print this off? I have a consultation tomorrow and it would be really useful but I've had a complete nightmare trying to print it! It asked me to log into my facebook account so the link ended up on my profile page for all to see!!

Yeah, I copied the questions out too for my daughter but I know what you mean, it's just not as good as having the entire guide with you, it's fab isn't it.
